Join Us For The Inaugural 2021 Transitions of Care Summit (June 15th-24th)

The patient voice and experience is an integral part of a successful transition from pediatric to adult care. The CNF Transitions of Care Summit is focused on ensuring that young adults living with a neurologic condition and those who support them, have the knowledgetools, and confidence to be able to make the process a success.  

The summit will be taking place between June 15th to June 24th with 7 live talks and 3 pre-recorded videos released to registrants at the start of the event 

Registration will get you exclusive access to the CNF Transitions of Care Summit hub on the CNF website that will include videos, tools, and recordings of the events. Registrants will receive an email prior to each event with the link to access the event on Zoom. Attendees are encouraged to have video on if appropriate but will stay muted throughout the event.

The Child Neurology Foundation is committed to empowering all young adults living with neurologic conditions, along with their caregivers, clinicians, and providers. We strive to improve the process of transition of care and eventual transfer to adult neurologic care. As a participant in the CNF Transitions of Care Summit you are committing to upholding the same values of professionalism, collaboration, and courtesy to ensure all participants at the event are respected. Attendees of the CNF Summit are not permitted to audio or videorecord sessions without the express written permission of CNF.

By participating in this event, I grant CNF the right to use any image, photograph, voice or likeness, without limitation, in its promotional materials and publicity efforts without compensation.
